LOTO Procedures

According to CFR 1910.147(c)(6)(i)
“The employer shall conduct a periodic inspection of the energy control procedure at least annually to ensure that the procedure and the requirements of this standard are being followed.“
Fall Protection
All inspections have a report submitted and provided to the company after the inspection is complete.
According to ANSI/ASSE Z359.2
“Fall Protection Equipment shall be inspected by the authorized person at least once at the the beginning of each eight-hour shift in which it is used to verify that it has not sustained any wear or damage that would require its removal from service. Fall protection and fall rescue equipment shall be inspected on a regular basis not to exceed one year (or more frequently if required by the manufacturer’s instructions) by a competent person or a competent rescuer, as appropriate, to verify that the equipment is safe for use.“

According to CFR 1910.146(d)(14)
“Review the permit space program, using the canceled permits retained under paragraph (e)(6) of this section within 1 year after each entry and revise the program as necessary, to ensure that employees participating in entry operations are protected from permit space hazards.“
NOTE: Employers may perform a single annual review covering all entries performed during a 12-month period. If no entry is performed during a 12-month period, no review is necessary.
Fire Extinguishers

According to CFR 1910.157(e)(3)
“The employer shall assure that portable fire extinguishers are subjected to an annual maintenance check. Stored pressure extinguishers do not require an internal examination. The employer shall record the annual maintenance date and retain this record for one year after the last entry or the life of the shell, whichever is less. The record shall be available to the Assistant Secretary upon request.“
OSHA 300 Logs
OSHA 300 Log summaries should be completed and posted by February 1st and stay up until April 1st each year. Some companies are required to submit OSHA Logs electronically, these must be submitted through the Injury Tracking Application (ITA) by March 2nd each year. According to CFR 1904.32(a)(1)
“Review the OSHA 300 Log to verify that the entries are complete and accurate, and correct any deficiencies identified.“
Chemical Inventory
These are recommended to be reviewed at least annually and the SDS Sheets should be kept as current as possible.
